When you changed your name

If the name of the insured person or a dependent has changed due to marriage, etc., please apply with the Society.

Change of name

Required documents:
  • (2) The health insurance card, Eligibility Verification Certificate (if issued)
  • Only when the name of a dependent or voluntarily and continuously insured person has changed
    (3) A document that shows the new name (such as a copy of both sides of the driver's license)
Only for applicable persons without a Myna health insurance card
