Change of name

Required documents:	(1)Notification of Name Change (Correction) of Insured Person
	(2) The health insurance card, Eligibility Verification Certificate (if issued)
	Only when the name of a dependent or voluntarily and continuously insured person has changed (3) A document that shows the new name (such as a copy of both sides of the driver's license)
	Only for applicable persons without a My Number Card as a Health Insurance Certificate (4) Application Form for Issue of Eligibility Verification Certificate Excel ・ PDF　Example
Destination:	Employees: To the company’s HR Retired persons and voluntarily and continuously insured persons: To the Health Insurance Society Click here for where to submit the documents.
Deadline:	Immediately after the name change
Applies to:	Insured persons and dependents whose names have changed
Address inquiries to:	Health Insurance Society
Notes:	After changing the name data in the HR system, send the required documents to the submission destination.
