Health checkups

Checkup flow

1. Before making an appointment (applicable to all)

  1. STEP1 Using the list of partner medical care institutions, choose your desired medical care institution, checkup course, options, and checkup date and time.
    • * Check the list for courses and options not available at certain medical care institutions, copayments, and other information.
    • * Contact us if it would be difficult to undergo a checkup at a partner medical care institution.

      Voluntarily and continuously insured persons, family members: Kenshin Club
    For complete checkup Stomach exams are optional (may be cancelled when making an appointment).
    For those requesting breast cancer screening Choose an examination method (either mammary ultrasound or mammography).
  2. STEP2 Choose an appointment method.
    A. Direct appointment with medical care institution (restricted to partner medical care institutions that accept direct appointments)
    B. Appointment through Kenshin Club (available for all partner medical care institutions)
    • * Some medical care institutions accept direct appointments.
      See the list of partner medical care institutions for institutions that accept direct appointments.

2. Appointment
(A: Direct appointment with medical care institution; B: Appointment through Kenshin Club)

3. After completing the appointment

Kenshin Club will send an email requesting you to obtain a medical care institution examination ticket.

  1. STEP1 Download and print the required documents as instructed in the email from Kenshin Club.
    • ■ Medical care institution examination ticket
    • Company/Health Insurance Society designated questionnaire (reply on the Web from the appointment system or in paper form)
    • ◆ If you would prefer to be sent the documents by mail, do not download them.
      They will be sent by mail automatically to those who did not download them no later than two weeks in advance.
    • ◆ They will not be sent by mail automatically if you downloaded them.
      If you are unable to print them, ask Kenshin Club to send them by mail.
  2. STEP2 Two weeks before the appointment date
    The medical care institution will send you the checkup documents, specimen containers, etc.
    Contact the medical care institution if you do not receive these.
    • ◆ Be sure to contact Kenshin Club if you need to change the appointment date or appointment details or cancel the appointment.
    • ◆ If you contacted the medical care institution directly, be sure to notify Kenshin Club of the details of the change or cancellation.
    • ◆ Change or cancel your appointment through Kenshin Club on the Web up to three weeks in advance. Do so by email or telephone thereafter.

4. Day of checkup (applicable to all)

  1. STEP1 Take the following documents, etc. to the medical care institution on the date of the checkup.
    • Medical care institution examination ticket
      If you forget to submit this document, you will not be eligible for the subsidy and must pay the entire cost yourself.
    • Medical care institution questionnaire, specimen containers
    • Company/Health Insurance Society designated questionnaire (reply on the Web from the appointment system or in paper form)
    • Health insurance card or similar document
      Submit if requested at reception.
  2. STEP2 Pay your copayment.

5.Checkup results received (applicable to all)

The medical care institution will send the checkup results to the home address you specified.

  • ■ If you are an employee and undergo a complete checkup, results of the specific health checkup will be sent at a later date, separately from the results from the medical care institution.
  • ■ Results of the specific health checkup will be sent to voluntarily and continuously insured persons and dependents (aged 40 or older) at a later date.

A: Direct appointment with medical care institution

  1. STEP1 Call the reception number of the medical care institution on the list of partner medical care institutions to make a provisional appointment (restricted to partner medical care institutions that accept direct appointments).
    What to communicate:

    □ Health Insurance Society name (“Hewlett-Packard Japan Health Insurance Society”)
    □ Your eligibility status (whether an employee, dependent, or voluntarily and continuously insured person)
    □ Name, address, and telephone number
    □ Requested date and time *Appointment must be made at least two weeks in advance.
    □ Requested checkup course and optional exams

    What to ask:

    □ Name of the medical care institution staff member with whom you spoke
    □ Details of the appointment: double-check with the medical care institution.

    • * You must pay yourself the entire cost for additional exams not eligible for subsidy or any courses not specified. Check the charges due.
    • * Use the same appointment method when making appointments for two or more persons.
  2. STEP2 At the time you make the provisional appointment, notify Kenshin Club of the appointment details on the Web or by telephone. You will be issued a medical care institution examination ticket.
    • Caution! The appointment will not be finalized unless you complete the procedure in Step 2.

    On the Web

    • (1) Access the Kenshin Club appointment screen from “Apply for a health checkup/complete checkup.”
    • (2) Enter the login code and password to log in.
    • (3) Choose the checkup course, options, and medical care institution for which you made the provisional appointment.
    • (4) Register by entering the required information in the form.
    • (5) Kenshin Club will send you an appointment finalization email.
      * Contact Kenshin Club if you do not receive the appointment finalization email.

    Information to enter under “Remarks”

    * Name of the medical care institution staff member with whom you spoke
    * Details of examinations, etc. requested that are not eligible for subsidy, if any
    * Any special support needed (e.g., wheelchair use, English-language service)
    * Log in again if you do not receive a confirmation email.

    Inform Kenshin Club staff of the following:
    • (1) Name and date of birth
    • (2) Medical care institution with which you made the appointment, appointment date and time, checkup course, options, etc.
    • (3) Name of the medical care institution staff member with whom you spoke
    • (4) Details of examinations, etc. requested that are not eligible for subsidy, if any

B: Appointment through Kenshin Club

  1. STEP1 Make an appointment on the Web or by telephone. (Available for all partner medical care institutions)

    On the Web * Make an appointment at least three weeks in advance of your requested checkup date.

    • (1) Access the Kenshin Club appointment screen from “Apply for a health checkup/complete checkup.”
    • (2) Enter the login code and password to log in.
    • (3) Choose a checkup course, options, and medical care institution.
    • (4) Register by entering the required information in the form.
    • (5) Kenshin Club will send you an appointment request completion email.
      * Contact Kenshin Club if you do not receive the appointment request completion email.
    • (6) Kenshin Club will send you an appointment finalization email.
      * Contact Kenshin Club if you do not receive the appointment finalization email.

    Information to enter under “Remarks”

    * Any special support needed (e.g., wheelchair use, English-language service)
    * Details of examinations, etc. requested that are not eligible for subsidy, if any (optional)
    * Names and dates of birth of others you would like to undergo the checkup with, if any

    • * When applying for two or more persons, do so separately for each person.
    • * Log in and try again if you do not receive a confirmation email.

    By telephone * Make an appointment at least three weeks in advance of your requested checkup date.

    Inform Kenshin Club staff of the following information.
    • (1) Name and date of birth
    • (2) Requested medical care institution, date and time, checkup course, options, etc.
    • (3) Details of examinations, etc. requested that are not eligible for subsidy, if any (optional)
    • * Provide the same information when applying for two or more persons.
  2. STEP2 Kenshin Club will make the appointment with the medical care institution.
    You will be notified when the appointment is finalized. Confirm the details.
