When you changed your name

If the name of the insured person or a dependent has changed due to marriage, etc. or you want to enroll a new family member as a dependent, please apply with the Society.

Change of name

Required documents:
  • (1)Notification of Name Change (Correction) of Insured Person
  • (2)The health insurance card (the card of the person subject to a name change or revision)
    (3) A document that shows the new name (such as a copy of both sides of the driver's license)
  • ※(3) is only necessary if the name of a dependent or voluntarily and continuously insured person has changed.
Destination: To the company's HR
Deadline: Immediately after the name change
Applies to: Insured persons and dependents whose names have changed
Address inquiries to: Health Insurance Society
Notes: If the name of an HPE, HPI, or DXC employee has been changed, change the name data in the human-resources system, and then send only (2) the health insurance card to the submission destination.
