You can cancel your eligibility as a voluntarily continuously insured person by canceling prepayment of your premiums. Submit an application form and a copy of each side of your Certificate of Eligibility for Unemployment Insurance Benefits to the Health Insurance Society. Your eligibility will lapse on the day after the premium due date for the month after the month in which you apply. You will be mailed the Certificate of Disqualification after processing by the Health Insurance Society. Your premiums will be deemed not paid in advance, and you will be refunded the balance after subtracting the monthly premium payments through the month in which you apply from your prepaid premiums.
