You cannot withdraw from voluntary continuous insurance to switch to National Health Insurance or to become a dependent on another family member's insurance. Therefore, you may be withdrawn if you lose your eligibility due to failure to pay insurance premiums by the due date (the 10th of each month or the following date when the date is Sat, Sun or holiday). In this event, you cannot choose the date on which you lose eligibility. Instead, you will lose your eligibility on the following date(the 11th of each month in principle), the day after the premium due date of the 10th for which you failed to pay premiums.
The procedures required involve submitting a Notice of Disqualification as a Voluntarily Continuously Insured Person along with your health insurance card to the Health Insurance Society within five days from the day after the due date in the month in which you wish to switch to National Health Insurance, without paying the premium payment in that month. After processing by the Health Insurance Society, you will be mailed the Certificate of Disqualification needed to join National Health Insurance.
Report to us on (1)Preferred date of Disqualification, (2)Changing your address or not, then we post necessary documents.
